This is a Clinical Case Manager/Recovery Support Specialist position in the substance abuse field. The minimum qualifications for serious inquiries is a bachelors degree in human services or related fields. Preferred to have some working knowledge of addiction. The clinical case manager/recovery support specialist position encompasses the following job duties:
Specific Duties and Responsibilities:
-Direct care with clients struggling with addiction and recovery
-Work with clients on an individual basis regarding their recovery from addiction
-Manage day to day client activities including meals, meetings and other appointments
-Aid the clinical treatment team in efforts to work with the client and help them recover
-Interview patients, referral resources, and significant others to obtain needed information to ensure treatment planning , and for discharge planning
-Complete a biopsychosocial assessment to include history, basic personality, and to identify problem areas of all assigned patients.
-Carry a case load up to eight patients.
-Develop Clinical Treatment Plans.
-Formulate and coordinate Continuing Care planning, and complete clinical discharge summary.
-Conduct individual and group therapies as assigned as well as complete clinical documentation for the patient record.
-Conduct patient education on addiction, recovery, and/or other areas as needed for provision of patient care.
-Maintain detailed and timely reports, records and notes of patient's progress.
-Participate in interdisciplinary treatment planning conferences, patient community meetings, clinical meetings and Program meetings.
-Interpret clinical findings and medical recommendations to patient, family referral sources and as indicated.
-Maintain contact with all referral sources. Participate in Referral development activities as needed.
-Maintain clinical licensure/certification and current clinical competence and uphold professional code of ethics.
-Other duties as Employer may assign from time to time.
