Job Description
Elmcrest Children's Center currently has a full-time Healthcare Integrator (HCI) position in its Oswego County, NY Bridges to Health Office.

The Healthcare Integrator (HCI) provides service coordination for enrolled children and families. They assess the need for services, develop service plans, link child and caregiver to identified services to assist in stabilizing the child's environment and advocate for child and family needs. Children enrolled in this waiver reside in the Oswego, Mexico, and Fulton area.

The Bridges to Health, (B2H), Waiver Program is designed specifically for vulnerable children who are or have been involved with the local Department of Social Services. This program supports children in the least-restrictive home or community setting providing opportunities for improving health and well-being of the children served and support permanency planning. The B2H Community-Based Waiver Program provides services to three populations of children who are at risk for higher level services.

These three populations include children with serious emotional issues, have developmental disabilities and/ or those children considered medically fragile. These children live at home and/or in the least restrictive home-like setting possible including foster homes and receive a variety of services where they live within their home community.
Job Requirements
An Individual who is highly motivated, independent, professional, organized, and has strong writing and communication skills preferred.

Master Degree in Social Work, Psychology, or Special Education; Health Care Practitioner; Registered Nurse; or related field and at least one year experience providing service coordination, information linkages and referrals for community-based services to children with special needs, individuals with disabilities or seniors. Will consider a Bachelor's Degree in a related field with at least 4 years experience in providing service coordination, linkages and services in the community.
