Turning Point Community Programs provides integrated, cost-effective mental health services, employment and housing for adults, children and their families that promote recovery, independence and self-sufficiency. The Pine Tree Gardens (PTG) program in Yolo County provides residential and day rehabilitation program services to 33 adults 18 to 59 who are diagnosed with a serious mental illness. Services include individualized treatment planning, educational and skills development groups, case management, individual counseling, crisis intervention, peer support and assistance with activities of daily living.

(Position titled "Personal Services Coordinator III" at the Pine Tree Gardens program)
Responsible for assisting clients in meeting their expressed goals while living in the community. Additional support in areas of medication management, housing, counseling and advocacy will be provided as needed. Provides leadership and direction for clients as assigned. Assists assigned clients to identify and progress toward achievement of their chosen goals. Advocates for clients in all areas of treatment and helps them apply for and receive services. Empowers clients in meeting their goals. Advocates for and assists clients and their families in accessing and receiving mental health treatment, medical care, housing, food and clothing as indicated. Develops individual service plans for each client. Meets regularly with clients at Turning Point sites or in the field, at job sites, homes, board and care facilities, skilled nursing facilities and other appropriate sites. Consults with or meets regularly with the client's family, care provider or other significant persons with client's consent. Coaches clients in developing age-appropriate social skills, money management, personal hygiene, and job readiness. Assists in establishing client's eligibility for Medi-Cal or other benefits and advocates for continuation of benefits when appropriate. Develop financial management plans for clients who receive Representative Payee Services and authorize appropriate expenditures. Interacts productively within the team in order to provide holistic psycho social rehabilitation services to an identified group of clients. Provides "on-the-spot" counseling that is both helpful to the clients and consistent with the philosophy of the program. Attends to the safety, health, and well being of clients. Completes paperwork as assigned in a timely manner. Performs other duties as assigned by the Program Director or Clinical Director. Provides support to other staff members as needed.
Graduation from a four-year college or university with a Bachelors degree in Social Work, Psychology, related field or license eligible. Four (4) years varied experience as a provider of mental health services is desired or varying degree and experience that meets qualifications for a Mental Health Rehabilitation Specialist. Additional qualifying experience may be substituted for education.
Valid California driver's license, current vehicle insurance and driving record acceptable to Turning Point's vehicle insurance company and the Vehicle Driving and Maintenance Policy. Possesses and maintains a safe and reliable form of vehicular transportation excluding motorcycles.
