POSITION SUMMARY: The primary function of the position is to provide community outreach to promote health and wellness.

DUTIES:
Monitor/assess health and social needs of Tribal members during routine home visits.
Notify appropriate medical and social providers of potential patient health/social problems.
Distribute and explain health education materials to the community, including developing and making individual and/or community presentations.
Provide routine home visits to pregnant teenagers; new teen mothers; or new single parents to educate and help prepare for parenting in the absence of an alternative program.
Coordinate the Health Fair and other relevant community program events or activities.
Deliver medications to home bound patients; may transport patients as needed.
Deliver Elder meals to home bound as needed.
Maintain a daily log of activities and complete a monthly report.
Assist nursing staff with preparing diabetic audits/reports as needed.
Assist nursing staff with screening patients in the Clinic as needed.
Coordinate home health services with Council approved agency.
Other duties as assigned.

QUALIFICATIONS:
Must have High School Diploma or GED.
Pass a criminal background investigation and drug screen
Have a valid Nevada driver's license.
Able to lift 40 lbs.
Medical background with current licensure as CNA, LPN or EMT strongly preferred.
Knowledge of health issues prevalent among Native people.
Dependable, able to work with community members on an individual and/or group basis.
Exceptional people skills, able to communicate effectively, a team player and exercise good judgment.
Respect all patients with compassion and sensitivity.
Strong organizational skills and able to work independently.
Will adhere to the strictest standards of confidentiality and patient privacy with an understanding of HIPAA.
Working knowledge of basic computer function/programs.
