LOCATION: Based in the Redding office of Passages Resource Center. This position works primarily with clients in the Family Caregiver Support Program a federally funded grant under the Older American's Act, and will cover Shasta and Trinity counties. In addition, may need to assist in Siskiyou, Modoc, Lassen, Glenn & Tehama counties. This program assists caregivers who are caring for older family members.

ESSENTIAL JOB FUNCTIONS: Conducts intake requests for services and carries out needs assessments for clients. Provides emotional support and consultation services to families of brain-impaired or chronically disabled adults. Develops an "action plan" for each client family and arranges for services to meet the required needs. Provides care coordination services to families. Participates in case review with Clinical Supervisor twice per month. Completes all required reports and documentation for required data collection. Maintains client confidentiality in all matters. Provides information to the public, to families of brain-impaired and/or chronically disabled adults and to other community agencies regarding CRC caregiver services. Conducts community education, outreach activities, caregiver retreat and psycho-educational activities and support groups as assigned by the Program Director and/or Family Service Manager. Completes other work assignments as directed by Program Director or Family Service Manager.

EMPLOYMENT STANDARDS: Education: Requires a Bachelor's degree in social work, counseling, or related field. Requires a minimum of the equivalent of two years full time professional experience working with the disabled and/or elderly population in counseling, support, advocacy, and education. Experience must demonstrate possession of strong communication, clinical, and counseling skills with demonstrated knowledge of the principles and practices of interviewing and active listening. Knowledge and Abilities: Ability to relate to older adults, and family caregivers, demonstrating sensitivity, active listening, and a respect for self determination. Requires the ability to work effectively with a variety of people to gather information and make decisions for client action plan; knowledge of community resources essential. Must have high ability to participate in and facilitate groups; have excellent verbal and written communication. Must have high degree of initiative with capability of working independently. Ability to function as a member of the Center team. Bilingual skills desirable, but not essential. Must have valid CA driver's license and own transportation and be willing to work as needed in nine-county service area. The successful candidate must be willing to participate in the DMV Employer Pull notice program as well as pass fingerprinting requirements.

COMPENSATION: Starting Wage: $18.63/hr + benefits. Benefits include employer contributions to a flexible spending account (i.e., a Section 125 cafeteria plan) from which elections may be made for health, vision, dental insurance; employer contributions to a 401(k) retirement plan (beginning year 2); employer paid life and disability insurance; vacation, sick leave and holidays. This is a 30 hour/week position.
