POSITION SUMMARY: Under the direct supervision of the Clinical Nurse Manager, this position is responsible for providing and assisting participants with activities of daily living. Provides restorative and supportive care for participants at the ADHC. Supports activity program as able. Provides support in memory impaired unit. Monitors health status and provides a safe environment for participants. Functions as a member of multidisciplinary team. Promotes professional working relationships with both internal and external customers. Adheres to and supports all organizational policies and procedures and standards. Promotes teamwork!

QUALIFICATIONS AND REQUIREMENTS:
High school diploma or equivalent is required.
Must be licensed as a Certified Nursing Assistant (CNA) in the state of Colorado.
Minimum of one (1) year of experience working with a frail, elderly or disabled population is REQUIRED.
Current First Aid/CPR Certification is preferred.
Knowledge of the principles, practices standards and techniques of a Certified Nurse Aide.
Knowledge of how to use and maintain in-home equipment (adaptive, etc.).
Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public.
Ability to work independently with minimal supervision.
Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
Must submit to and pass a pre-employment drug/alcohol screening and criminal background check.
Must have a current Colorado Driver's License and driving record must be in compliance with Company policy.
Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records.
Must be free of communicable diseases that can be readily transmitted in the workplace. TB test is required within first 7 days of employment. Hep B vaccinations are offered but not required.
Operates in full compliance with all applicable federal, state and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.

DEPARTMENTAL (JOB SPECIFIC TASKS):
1.Must maintain calm and professional demeanor.
2.Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
3.Creates and participates in group(s) and one-on-one activities in memory impaired unit.
4.Monitors and assists participants with meals at feeding table.
5.Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.
6.Assists the drivers with the loading and unloading of participants from vehicles.
7.Demonstrates dependability through consistent compliance with scheduled work hours.
8.Participates in participant's care planning through cooperation with multidisciplinary teams.
9.May rotate into in-home services as a homecare CNA as applicable and deemed necessary by the Clinic Operations Manager.
10.Assist with ROMS sheets and food reimbursement program as needed.
11.Participates in Quality Assurance programs.
12.Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
13.Maintains courteous, helpful and professional behavior on the job.
14.Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.
15.Consistently cooperates and supports organization in problem solving issues.
16.Participates in continuing education classes and any required staff and training meetings.
17.Responsible for maintaining professional affiliations and any required certifications.
18.Other duties as requested/assigned

The employee may be required to endure extremes in temperature and may work in hazardous environments where personal protective equipment is required. May be required to transfer participants needing total assist with proper technique and safety equipment if necessary. Requires the ability to use and repair department equipment. Requires the ability to lift/carry up to 65 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics.
Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to within normal range, with or without reasonable accommodation. Must be able to communicate effectively in verbal and written form with all levels of personnel within and outside of the organization.
Pressure Factor - Requires working under stressful conditions. Moderate pressure to meet scheduled and recurring deadlines.
Environmental Conditions -- Working conditions may be noisy with fluctuating indoor temperatures. Must have the ability to work under moderate pressure in order to meet scheduled appointments or deadlines while dealing with frail, disabled and/or confused participants/clients. Subject to individuals who may have the potential for physical or verbal aggression. May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and other conditions common in a clinic environment. Subject to unpleasant odors.
