Main Function:
Provide support to a child and his/her family in the framework of a team oriented approach that will foster skill building and quality of daily living in the home and in the community.

1.Provide supports, which can include personal care assistance, consistent with individual plans.
2.Encourage and implement participant choice within program guidelines.
3.Develop supportive relationship with the child, his/her family and their circle of support.
4.Complete documentation based on program requirements following established agency and
government regulations.
5.Maintain professionalism with the child, family, service providers and in keeping written and verbal information confidential.
6.May provide support to children who have significant behavioral needs and able to implement behavioral support plans as indicated.
7.Responds pro-actively to crisis situations and within agency guidelines.
8.Attend training programs to develop needed skills as requested.
9.Attend team meetings as requested.
10.Maintain compliance with Agency policies and procedures.
11.Perform related duties as assigned by supervisor

Education & Experience:
Excellent verbal and written communication skills, including ability to effectively communicate with family members and other service providers. Ability to work with minimal supervision within parameters of instructions given, prescribed routines, and standard accepted practices. High school diploma or GED required, Associate degree preferred.

Conditions of Employment:
Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Ability to safely and successfully perform the essential job, including meeting qualitative and/or quantitative productivity standards. Ability to maintain regular, punctual attendance consistent. Must be able to lift and carry up to 50 lbs, walk, climb stairs, stand, stoop, reach and have good eye/hand coordination. Must also have good dexterity and ability to read and write. Must be able to listen and communicate clearly. Must work with others at least 75% of the time and able to sit in meetings for at least 1 hours at a time. May need to be outside in extreme climates (hot, cold, wet).Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York Office for People with Developmental Disabilities, Chapter 575 of the Laws of 2004. Must participate in successful database checks through the State Central Registry of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specialty Designated Nationals.
