Senior Alternatives, a private Care Management and Homecare company, is looking for a part-time Care Manager to join our professional team. Territories include Alameda and Contra Costa Counties. Our client population includes seniors, adults and families affected by chronic medical and mental illnesses. At Senior Alternatives, we use a multi-disciplinary team approach utilizing Social Workers, an RN Care Manager, and a Gerontologist.
The ideal candidate for the open position would have a Master's in Social Work (MSW) or RN, and have at least 5 years experience working with seniors and adults. This position offers an ideal work environment for someone who wants autonomy, opportunity, flexible schedule and competitive compensation.
Responsibilities may include: assessment, care planning, care monitoring, crisis management, education, counseling, and support. Some on-call work may be required.
