MAIN FUNCTION: The Registered Nurse is responsible for directly providing patient care. To assess, plan, implement & evaluate the nursing process through Firstat Nursing Service's policies, procedures & standards. Care will be provided within the framework of the nursing services' philosophy.
ROLES & RESPONSIBILITIES
1.Performs the initial nursing assessment of patients to determine if the patient meets admission
criteria. Assists in assignment of personnel, this includes care plan. MD orders to provide care.
2. Assists the physician in developing the initial & ongoing plan of care. Revises the plan of care with
the physician as necessary based on the on-site evaluation of the patient's conditions & needs.
3.Develops a care plan based on the nursing process, reviews & updates care plan as necessary.
4.Orients staff members to each home care case as applicable.
5.Conducts supervisory aide visits per Medicare/State/Federal regulations.
6.Will obtain orders from client physician for recertification's, transfers and discharges.
7.Establishes & maintains communication with the physician, pharmacist and/or referral sources regarding patient's progress.
8.Counsels & instructs the patient/family in the set-up & administration of home infusion therapies. Provides ongoing clinical monitoring.
9.Monitors patient's inventory of pharmaceutical products & supplies, as applicable.
10.Attends case conferences & in-service education programs.
11.Ensures coordination of patient services.
12.Prepares clinical notes & enters them into the clinical record (AXXESS) on a daily basis. Reviews documentation on a continual basis to ensure compliance with Firstat Nursing Service's documentation standards.
13.SOC, Recertification, Resumption of care, Transfer and Discharges will be paid upon completion in full and in QA.
14.Documentation that is not in AXXESS or in Save mode will be paid upon completion.
15.Administers medications & treatments as prescribed by the physician & the state's scope of professional nursing practice.
16.Reports to the Director of Nursing and/or the physician any changes in the patient's condition & needs.
17.Adheres to administrative & patient care policies including the implementation of patient's rights & responsibilities.
18.Performs on-call duties as assigned.
19.Performs additional duties & responsibilities as deemed necessary.
